We encourage reports on all incidents of concern, even if only partial information is known.
Anonymous reporting is available for most forms, although anonymity may limit the university’s potential for follow-up action.
All reports are treated with privacy and confidentiality in accordance with University policy and local, state, and federal laws.
Concerns about a Student’s Well-Being
Use the form below to report a concern regarding a student’s well-being. If it’s an emergency, please contact Campus Safety at 585-594-7777 or dial 911.
Examples of concerns may include:
Reports will be received by the Dean of Students, with responses coordinated by the University’s Care Team (“R-Care”).
Sex-Based Misconduct, Discrimination, and Harassment / Title IX
Use this form to report incidents or concerns that may involve any form of sex-based discrimination involving anyone connected to the University, including:

Bias, Discrimination, and/or Harassment
Use this form to report incidents or concerns of potential bias, discrimination, and/or harassment by or against a member of the University.
Examples of concerns include any actions against someone with a protected class status that limits their employment or participation in educational programs, including:

Use this form to report other concerns or incidents involving an RWU student, employee, guest, or vendor/contractor. Examples may include:
