Immunization & Health Requirements

Incoming Students

HEALTH & IMMUNIZATION REQUIREMENTS MUST BE SUBMITTED BY THE FOLLOWING DATES:

Fall Semester - July 1st

Spring Semester - December 15th

Failure to submit required information may prevent you from being cleared for residence, class attendance, and sports participation.

REQUIREMENTS FOR ALL STUDENTS

  1. Required Immunizations (MMR, Meningitis) *Supporting documentation required. You may use the provided form along with a physician’s signature OR provide an official copy of your immunization history from your doctor’s office.
  2. Health History Form
  3. Consent for Treatment & Authorization for Emergency Services

  1. Tuberculosis (TB) screening questionnaire
  2. Proof of active health insurance

All new and transfer student-athletes are required to submit health and immunization requirements, as outlined by the NCAA and New York State Public Health Laws before beginning any intercollegiate athletic team participation for Roberts Wesleyan University.

In addition to the general student health and immunization requirements, all first-year and transfer student-athletes trying out for an intercollegiate athletic team are required by the NCAA to provide proof of:

  1. Physical Examination completed by their personal physician or qualified health care provider dated within the 6 months preceding any sports participation.
  2. Active Health Insurance
  3. Sickle Cell Disease/Trait Test Results
    • The following options are available for obtaining sickle cell disease/trait status:

      Students who are currently NYS residents but who were not born in NYS should contact the newborn screening program in the State they were born for results.

Returning student-athletes must complete a Medical History Questionnaire (found in your student portal) every year, within the 6 months preceding sports participation, as long as there has been no change in health, including injury.

If there has been a change during the past year, a physical examination is required.

  • Physical examination (dated within 1 year of clinical placement).
  • Tuberculosis (TB) screening, varicella vaccination, annual flu/covid vaccination/declination, hepatitis B vaccination/declination, current Basic Life Support (BLS) certification, active health insurance

New York State Public Health Law 2165 mandates ALL students enrolled at institutions of higher education ARE REQUIRED to provide evidence of vaccination against measles, mumps, and rubella, if born on or after January 1, 1957. For more information about MMR requirements please review the New York State Public Health Law Immunization Requirements.

Immunity can be demonstrated in one of the following ways:

  • Documentation of 2 MMR vaccines (measles, mumps, rubella)
  • Documentation of measles vaccination (2 doses), mumps (1 dose), rubella (1 dose)
  • Titer tests (blood tests) showing immunity to measles, mumps, and rubella

New York State Public Health Law 2167 mandates ALL students, regardless of age, to provide proof of meningococcal vaccine (dated within 5 years) or a signed declination statement. For more information about meningitis vaccination requirements please visit the New York State Department of Health website.

Per New York State Public Health Law: No institution should permit any student to attend the institution in excess of 30 days without complying.

Former Students Immunization Record Release

New York State law requires us to maintain medical records for six years from the date of last visit.

If you need a copy of the immunization records you provided to the Wellness Center sent to another institution, you may be able to access a printout of the vaccinations you provided to us through the Student Health Portal.

If not, please contact the Wellness Center to request your information. An uploaded copy of your photo identification is required to complete the request.
